Tirzepatide

Also known as Mounjaro, Zepbound

Dual GIP and GLP-1 receptor agonist.

Medical status: Approved for type 2 diabetes, weight management, and obstructive sleep apnoea in obesity.

Also commonly used for (off-label)

  • Aggressive weight loss
  • Appetite and blood-sugar control

Off-label and research uses are what people report, not approved indications. Talk to a clinician before using any of them.

How it's taken (route)

Documented dose
2500–15000 mcg
Typical frequency
Once weekly, same day each week — hold each dose at least 4 weeks before increasing
Half-life
~5 days
Common vial sizes
15 mg, 30 mg, 60 mg
Regulatory status
FDA-approved prescription medicine.

Dose on a U-100 syringe (2500–15000 mcg)

VialBAC waterDraw
15 mg1 mL17–100 unitslarge volume
30 mg1 mL8.3–50 units
60 mg1.5 mL6.3–38 units
60 mg2 mL8.3–50 units

On a U-100 syringe, 100 units = 1 mL, and the syringe holds 100 units total. Most people keep a single subcutaneous injection under about 50 units — bigger volumes are usually mixed stronger (less water) or split into two shots. Only mixes that fit in one syringe are shown. Always double-check with the reconstitution calculator.

What it does

A dual agonist that activates both GLP-1 and GIP receptors, the two main gut hormones that manage appetite and blood sugar. Hitting both tends to produce more weight loss than GLP-1 alone. Approved as Mounjaro and Zepbound. What people look into it for: • Large average weight loss in trials (often around 20% at higher doses) • Strong appetite reduction and fewer cravings • Excellent blood-sugar control in type 2 diabetes • Improvements in blood pressure, cholesterol and sleep apnoea Doses step up slowly to limit nausea and stomach upset. Protein intake and resistance training matter for keeping muscle.

Mechanism

Dual incretin agonism improving insulin secretion, satiety and gastric emptying.

Storage — lyophilisedwhat's this?

Refrigerate 2-8C, protect from light.

Storage — reconstitutedwhat's this?

Refrigerate 2-8C, use within ~28 days.

Reconstitution noteswhat's this?

Common mix: 30 mg vial + 1.5 ml bacteriostatic water = 200 mcg per unit, so 2.5 mg is about 12 units, 5 mg is 25 units and 15 mg is 75 units. A 60 mg vial + 2.5 ml gives a similar draw.

Stacks well with

Popular combinations reported in the community and research literature — not a recommendation. Adding peptides multiplies side-effect risk; check with a clinician.

  • Diarrhoea
  • Vomiting
  • Decreased appetite
  • Injection-site reactions

Contraindications noted

  • Medullary thyroid carcinoma history
  • MEN2

Research notes

Dose escalation over months is standard in the approved label.
